The DD-WRT firmware is probably in the the same boat. You can't have Milkfish manage the remote end for everything. It can handle internal calls, as it keeps track of extension registrations, but outgoing calls go back to 3CX, it would seem, then back to the remote extension, and you might have to live with that.
